AGENT'S TERMS AND CONDITIONS :-

Kindly read and understand all the terms and conditions and then tick them to proceed.

 

The applicant for streamyx can be :-
1. the owner of the TM telephone line, documents required:- photocopy of owner's NRIC
2. the user of the telephone line i.e. he/she is not the owner of the TM telephone line
documents required:- photocopy of owner's NRIC or owner's both new and old NRIC number, photocopy NRIC of TMNet streamyx applicant, authorization columns are signed by both parties.

   

For business applicant, please submit the original TMNet streamyx form, chop and sign, applicant's NRIC and Company Registration form (either D,B or A for Enterprise, form 9 or form 13 for Company Sdn Bhd/ Bhd). For Government and NGO applicant, please submit the original TMNet streamyx form, chop and sign, applicant's NRIC and a Letter Head with approval from the head of department.

   

Applicant not being black listed by TM and TMNET.

   

Please fill in applicant's old and new NRIC number, especially if their birthdate is before 1970. Persons who are above 35 years old.

   

For self installation package (package without modem), agent must monitor and inform their customer when it is ready for installation. For streamyx-in-a-box (SIB) package, agent is required to collect the modem from us. We can also courier to your customer @ a fee of RM14.00. They will get it in 3 working days.

   

For self installation package, agent has to inform CITSASB by email immediately if the status is 'kaji siasat' in the system so that we can assist to get streamyx for your customer and prevent auto-activated account.

   

For self installation package, agent must inform customer that their account will be auto activated within 3 days after the status is set to 'Peringkat Awal' in the system. Once activated, TMNET bill will be generated and the account cannot be cancelled anymore.

   

For self installation package, in the remark column, please write "CD requested and collected" or else, TMNET installer will be sent to assist your customer in the installation and RM88 will be billed in their first bill.

   

Once login ID is registered and accepted, it cannot be changed.

   

Please ensure and verify that all the details keyed-in are correct before submitting. Penalty will be charged accordingly for wrong package registered. TMNET has not finallised the penalty yet but as an example of the penalty, SIB packages is non-cancellable, hence the agent will be billed 12 or 24 months (according to the package subscribed) on behalf of their customer.

All TMNET streamyx submission will be eligible for claims only if all supporting documents are in order.
